How to unlock the Streaker achievement in Kingdoms of Amalur: Re-Reckoning - Definitive Guide
This achievement is unlocked upon speaking to any NPC in the game after unequipping all weapons and armor. The first opportunity is in the prologue. You meet Encel, a gnome. After the scripted dialog with him concludes, press . Choose inventory -> weapons and unequip the sword by pressing . At this point, you have not acquired a secondary weapon. Go to armor and unequip all armor. Your beginning armor is a tattered shirt & ragged leggings. If you have equipped any dead enemy drops, remove them. Back out and press to talk to Encel. The dialog does not matter and the achievement unlocks as shown in the below video:
Note: I had originally assumed accessories must also be unequipped if the player chooses to unlock this achievement after acquiring accessories. @So This It in the comments verified that accessories, such as a ring, may be worn.

accessories don't seem to be a problem. I just unlocked with a ring equipped.

Your comment made me think that, perhaps, the achievement description should be taken literally. When I get the chance, I’ll test if the achievement can be unlocked by an unclothed character carrying a weapon.

In order to unlock this achievement you must not be wearing any armor or accessories. Make sure to unequip accessories in their own separate menu.
Can be completed as early as the first conversation in the game during the tutorial. Simply take off all armor and accessories before the first gnome you speak to runs up to you after saving him from the Tuatha in the Well of Souls.

You don’t need to unequip accessories, just armour.

When you reach the first town, simply go into your inventory (pressing for the main menu). Select 'armour' and then dequip everything. Talk to any random NPC without armour on for the trophy to pop.
UPDATE:
Two gamers have confirmed that they didn't get the trophy til all weapons were dequiped too.
I've discovered on a seperate site that the original solution may be down to a glitch where as long as u take off your main hand and don't press whilst going for the trophy, the trophy pops as the game thinks you've dequiped everything.
To Summarize, just take everything off to make sure it works.

You must remove everything for the trophy to pop. All armour, weapons and accessories must be removed.

This can be done straight away in Gohart. Go into your menu and select every piece of armor and accessory to remove them. Once you are down to your underwear, speak to anyone in town to unlock the achievement.
