It's pretty self explanatory but I did want to mention that going from normal straight to Legendary does not unlock this achievement like some other "finish on X difficulty" achievements do.
You will keep your levels and gear but the game will not equip your good weapons so you'll start the first battle doing 1 damage. Use Adachi's arrest on your first 3 enemies and then you'll be able to equip your proper weapons.
You also can't change jobs until chapter 5 so you may want to secure good gear for your starting jobs in premium adventure before moving to NG+

Completing the game+ on Legendary difficulty does not award the Hard difficulty trophy. Fortunately, you don't have to complete the campaign on hard mode again, just change the difficulty in the final battle and set it to hard in options.
