500 counters throughout the game, easy if you are going for the Hard story mode achievements. If not, you will be pretty close towards your goal of 500...
Few quick tips:
Hit the RB button (Default controls) when the little icon appears on the screen before any attack. Watch the icon the first few times to figure out when to press the button, but after that, you will be doing it on instinct.
If you spam the RB, it wont work, so you have to press it at a certain time. If you press it too late / early, it will let you know via in-game prompt.

You may get this either in Story Mode or in Pride Battle mode. Most likely you will earn this in Pride Battle mode. To counter press 
when prompted on the screen as your opponent is trying to hit/grapple you. A successful counter is when you block/push away your opponent. Do this 500 times over course of play in one mode and the trophy unlocks after the 500 counter.
If using two controllers, have controller 2 tap 
and your controller (controller 1) press at the same time to speed up getting counters.
