A counter is a blocking move you can do when an opponent is kicking, striking or grappeling you.
You will se a prompt to press RB, do that 50 times in the same match, and the achievement is yours

To counter press 
when prompted on the screen as your opponent is trying to hit/grapple you. A successful counter is when you block/push away your opponent. Do this 50 times in one match and the trophy unlocks after the 50th counter.
If using two controllers, have controller 2 tap 
and your controller (controller 1) press at the same time to speed up getting counters.
