Thanks for this guide! Since I've completed all DLC and side missions, I don't have access to other mission related tips. This guide let me complete several achievements including this one.
Base Game
- Sending a Message - chain 3 or more brutal takedowns. Punch then brutal TD the first, wait until the 2nd nearest cop to get back and punch then brutal TD again, then wait again for the 3rd.
- CLosed Casket - 50 brutal takedowns. Since cops just keep pouring, you can keep chaining brutals.
Faster, Baby DLC
- Danger Close - kill 30 with Proximity Mine. Since cops were sometimes clumped, I get extra kills.
Stones Unturned DLC
- Spotter - kill 30 enemies using Sniper Support. Again, if there are multiple enemies in a bunch, sniper kills each one for 1 activation
- Big Fat Party Animal - I use the Hartmann 7.62 and wait for at least 4 to show up and use slow-mo for the Pop, Pop achievement also
Sign of the Times DLC
- Pop, Pop - See above. Kill the first 5.

Go to the police station in French Ward.
Throw a "Screaming Zemi" doll in the garage and lure all cops together. (Select the Screaming Zemi, and hold and release ).
The Screaming Zemi distraction doll is available as part of the Arms Dealer perk from Cassandra. You can find it under 'explosives'. (Hold at any time in the game and choose 'Arms Dealer'.)
Use a rifle or pistol for easier headshots and QUICKLY shoot them in the head.
I used the Hartmann .30. (Also available from the Arms Dealer).

In order for the police officers to count as enemies, you have to wait until a message appears on the screen like "The Police have witnessed a crime". Otherwise those headshots won't count towards the achievement

I tried changing aim assist from high to off and it did nothing. Uninstalling the game probably won't change a thing, the new save might... Trying to do this offline and then connecting also might help.

Bottom Line Up Front: This is an alternative approach to the conventional way of shooting five enemies on foot. Lincoln makes the five headshots while in a vehicle.
Advantages: No expensive Screaming Zemi Distraction Doll is required. Targeting is performed by the game so skilled player controlled targeting requiring hand and eye coordination is not required.
Disadvantages: The player must assemble at least three police cars in close proximity to Lincoln. This is a challenging task. Although skilled hand and eye coordination is not required to the extent of the on foot approach, the player must be comfortable in a vehicle and be familiar with the mechanics of targeting and shooting while in a vehicle.
This is a very difficult achievement no matter how you do it. As of this writing, the TA ratio is over 5. If you are having difficulty doing it on foot, you might try doing it in a vehicle where the targeting to the head is done automatically by the game.
This in-vehicle approach to the achievement gives players an alternative to the on foot approach. The on foot approach, generally (but not necessarily) uses a Screaming Zemi distraction doll to cause the enemies to congregate close together around the screaming Zemi. It requires skilled hand and eye coordination to quickly take the five headshots in rapid succession. I prefer the in-vehicle solution to the on-foot solution. The Screaming Zemi distraction doll is expensive and I knew I would need some costly practice if I tried to get the achievement on foot by having the victims congregate around a thrown Screaming Zemi distraction doll.
Even though the head targeting is performed by the game, the in-vehicle technique has its own difficulties. When shooting, you must have at least three police cars close to you. There are two officers per car so, to get the five headshots you need for the achievement, you have to be chased by three cars.
Moreover, the cars must be close to you. If the cars are too far away, the time per shot is increased. So, even if you make the five headshots, they may take over five seconds to perform. So, although the in-vehicle technique does not require the skilled hand and eye coordination of the on foot approach, it challenges the player with managing the chase in such a way that Lincoln is boxed in by at least three chasing cars.
The achievement can be unlocked wherever you can get three police cars to 'corner' you where you have the fields of fire to perform five headshots. The below video selects the French Ward area. The street running by Father James's Church, a safehouse, is wide enough to accommodate Lincoln's car and the three chasing vehicles. I could not get three vehicles to reliably chase me when provoking the chase from inside the car. So, Lincoln provokes the chase on foot at the French Ward police station. He assaults a police officer, gets in his car parked nearby, and drives to the target point opposite Father James's Church.
Having equipped a weapon (press and select the handgun), Lincoln cycles the targeting from tire to engine to headshot by pressing twice. When three cars have boxed him in Lincoln shoots the six occupants by pressing six times. Make sure that the green dot has appeared on the target's head, otherwise the shot will not count as a valid headshot. If you shoot too quickly, the automatic targeting will not have switched to the next headshot. In that case, the second shot will be a wasted shot into the dead body of the first target.
Some key points:
--Lincoln will take damage. He uses a pickup truck in the video because it provides more protection than a car. The specific truck in the video is the Bulworth Buckliner 75 that is a random spawn near Vito's place. Although not used in the video, a tac vest is recommended.
--Do not overdrive the chasing cars.
--The hardest part of this technique to get the achievement is allowing three police cars to corner you where you can make the headshots. There is some randomness to the chase so you rely, in part, on luck. Because of that, make sure you have internalized the elements within your control--understanding your planned route, drawing your weapon (), cycle the targeting to headshots (-), shooting at least five headshots and preferably six (-----[optional]).
--I recommend the Elling 9mm pistol for its high rate of fire, large magazine, accuracy, and stability.
It took me about an hour of practice to learn how to manage the chase. Fortunately, it takes place near the safe house so one can revert to the last checkpoint should the chase not turn out as desired. In my experimentations, I first tried provoking the chase while in the vehicle but could not achieve the consistent outcome I was looking for. If someone can figure out a reliable way of provoking the chase in the vehicle, this would be preferable to the method used in the video. Please add a comment if you have some insight on improving the chase segment of this solution.
If you are having problems with the chase:
There are some story missions and side missions with a built-in chase. My approach to the game was to get the technical/non-story related achievements out of the way so they wouldn't be a distraction once I tackled the story in earnest. In this case, though, if you are having trouble engineering a chase where at least three vehicles pile up close to you, you might want to consider using one of the missions that includes an intense chase.
A very good one is the Steal Police Van which is the final part of Cassandra's Side Mission, .45 In My Hand. Normally, you would want to take out the cops before stealing the van so nobody notices you driving off with it. But, if you make a commotion you will have a bunch of police cars that easily close in on the slow van. Moreover, the van affords you enough protection so you will survive long enough to get off the shots you need for the achievement. Remember the enemy vehicles have to be right next to you in order to be close enough to pull off the five shots in five seconds.
A good story mission is My Name's Lincoln Clay... The mission begins with Santangelo ambushing you with his goons regardless of whether you are on foot or in a vehicle. If Lincoln is in a vehicle, the ambush consists of quite a few cars. It is best to be in the Armored Majesty for survivability that unlocks when you complete the Kill Tony Derazio (part of the Everyone will Notice) mission. You'll want the cars to close in on you, ram you, and surround you. So, you will take quite a bit of damage. However, once the enemy cars have piled around you, you will be able to get the five headshots in five seconds. Once again, a reminder that the enemy vehicles have to be right next to you in order to be close enough to pull off the five shots in five seconds.

It might help if I explain my tactics to start the chase. First, I hide close to the cops and whistle. One starts over to Lincoln. Lincoln moves away to a further hiding place because I don't want the other cops to be close enough to see and shoot at Lincoln. Lincoln confronts the cop. He must see Lincoln so that the crime will register.
As soon as the announcement about a crime being witnessed appears, Lincoln runs to his car which is parked on the divided boulevard facing against traffic. Lincoln then draws his weapon by pressing and the white targeting reticle appears. I don't change the targeting until I am cornered. That's when I cycle the targeting from tire to engine to head by pressing twice.
A couple of things to improve survivability. If you have unlocked the Armored Majesty, that will give you a lot more protection than the pickup truck I use. Additionally, if you have been cultivating Cassandra favors and have unlocked armor vehicle options that will help, too, when you purchase them. Also, it helps to equip a tac vest that I do not use in the video.

This can be done at any point during the game, after progressing through the story far enough to unlock the screaming zemi. The screaming zemi is not required, but makes this much easier. Find a grouping of enemies and use the screaming zemi to lure them to a location, get close and simply headshot them all.

I've tried the Zemi-method a few times but couldn't get the trophy that way. Then I've found another method which is shown in that video from Studio 308 Games. Got the trophy on my first try.

So with the addition of the third dlc " Sign of the times" this achievement is much easier. First off you can now use a slow down mode when shooting, by aming and pressing the y button. Second during the dlc mission " All for the blessed" during one part you are on a balcony watching a large group of enemies without them knowing you are there. Just throw a voodoo doll in front of them, aim, hit the slow down mode and fire away. I got it with the silenced pistol. If you don't get it reload the checkpoint and you will start right before the area.

Just wanted to point out another very important fact everyone forgot. After trying everyones methods with no luck i decided to put the aim assist off and used the garage method. After tossing the zemi and waiting for the cops to bunch up i pointed my rifle and waited for them to become hostile. Then i killed them and even killed a cop by shooting him in the chest and got the achievement anyway. If the aim assist isnt necessary i dnt know but it helped me.

Just to note, it is possible to do this against enemies instead of cops. I was having trouble with the cops and got the achievement In Frisco Fields towards the end of the game. In the NW portion of the map where you have to go to kill someone, you will drive up to a house where the enemies are in the backyard. Throw a voodoo doll between the house and the fence, and it forces all the enemies into a small place. Fire away and you should be able to hit at least five of them with headshots

Impossible to unlock this achievement with policemen , then i used a group of ennemies while a traffic attack , with a Zemi puppet and the DECON , this gun have a silencer , like this The enemies will remain surprised and stay motionless long enough to perform your 5 headshots.

Got this in the last part of the ".45 in my hand" mission where you raid a weapons warehouse. There are 4 guys talking to each other inside next to a big crate with a fifth really close, I just came from behind the crate and got them all in a second with the Stromer .223
Edit: If you have the "Stones Unturned" DLC this is trivially easy in the "Out-of-towner" bounty hunter mission, there are 5 guys in a line at a bar and if you mess up just kill the bounty and restart checkpoint to try again. I did the 4 kills in slo-mo 10 times achievement here.

This can be done during the “Confront Pete Santini” mission. When you trespass go to the trailer on the right and crouch by the door. There should be 2 guards close and a sentry guard outside and at least 2 inside nearby. Throw a voodoo doll by the door of the trailer you are in and they will all arrive there. Go in slow motion (need the dlcs for this) by pressing why and using a pistol headshot 5 ppl. This took me 4 attempts to do but a couple tries and it should work.
This might be a good location for getting this achievements

Got this one while doing one of the main mission that you need to kill one of Junior's thieves in Tickfaw Harbor.
Use a Screaming Zeni to lure enemies to one spot, then quickly pop their heads. The Screaming Zeni is unlocked after obtaining Cassandra as an Underboss. It also helps to upgrade your pistol or automatic guns accuracy, which you can do by increasing Cassandra’s Earn.
