How to unlock the Shh, shh achievement in Mafia III: Definitive Edition - Definitive Guide
Edit 30 Apr 2017 - Caution: This achievement had been glitched but, based on the number or recent winners, the problem has been solved. If you have difficulties refer to the strike-thru comment above.
This should come naturally in the normal progression of the game because it is often advantageous to perform stealth takedowns. However, you may want to grind it whether for instant gratification or to practice the technique. Moreover, since the achievement had been glitched, you may want to satisfy the conditions to unlock it as soon as possible to verify that it will not present a problem.
Stealth Takedown: A stealth takedown is a lethal or non-lethal takedown performed when the target has not been alerted to Lincoln's presence. This can be the case where Lincoln sneaks up behind an enemy while crouching. It is, perhaps, most easily performed when Lincoln is in cover and whistles. A nearby enemy will come to investigate and can be caught unawares with the stealth takedown.
Technique: When Lincoln gets close to an enemy who is unaware of his presence, usually, but not always, the symbol will appear on the enemy signifying a stealth takedown can be performed. However, this indicator is not perfectly reliable and so practicing in a controlled environment may be of benefit to perfect the timing. You will quickly learn when a stealth takedown can be performed and will be able to execute it regardless of whether the prompt appears or not. The takedown itself is performed by tapping or holding .
The use of for a stealth takedown differs from its use against an alert enemy. In the latter case, tapping is a light attack and holding is a heavy attack (with the option of performing a brutal attack with the right timing). Against an unalert enemy, all uses of are the same--a stealth takedown with corresponding animation. As Mobius Evalon appropriately advises, the easiest technique is a light tap of the B button at the appropriate time.
Grinding the Achievement:
The achievement can be grinded in many places should players want to unlock the achievement without waiting for it to pop in normal game play. The below video shows a controlled, set-piece type environment where stealth takedowns can be practiced. It is early in the game. Cassandra has become Lincoln's ally and the Way of Flesh mission is underway. It is during the Prostitution Racket sub-mission. At that point in the game, members of the Dixie Mafia have started loitering in pairs at certain locations.
The grinding location has Lincoln do a stealth takedown at a corner on one of the loitering Dixie Mafia members. Then he goes into the First Baptist Church complex and performs two more stealth takedowns. When Lincoln returns to the street corner where he started, the loitering Dixie Mafia duo will have respawned. So, Lincoln can go back and forth doing three takedowns between the street corner and the house or garage in the First Baptist Church complex.
The First Baptist Church complex plays a part in the Smack sub-mission. So, the location becomes unavailable partway through the Smack sub-mission. Even though it becomes unavailable early on, it is still a good location. There is a Tac vest and a First Aid box in the house or garage near the Church. So, Lincoln can take care of his health. Even though the takedowns are stealth takedowns other enemies are alerted and will attack Lincoln. So, should Lincoln take damage, he can reconstitute his health and protection. The Tac Vest, if used, respawns when Lincoln goes as far as the street corner with the Dixie Mafia pair.

@AcaelusT - The game hub has changed over the years and is a little more complicated to get to than it was when the game first came out. Press on the controller & highlight the game. Press and choose Game card. At the bottom of the game card screen, select, "Go to official club." At the top, tab to "Progress." On the left hand side of the progress screen, you'll see Achievements, the default, and Stats. Go to stats. This is the game hub screen.
When Mafia III first came out, you could go to it directly. Now, it's rather convoluted and you have to get to it in the way I described above.
The current game hub shows four stats: Time played, cash earned, enemies killed, and enemy takedowns performed.
The importance of the hub is you can see if the counter is working. If cash earned, enemies killed, and enemy takedowns performed do not increase as you play the game, it means the counter is not working.
Unfortunately, if the counter is not working for one achievement, it often is broken for all achievements that rely on the counter. The first remedy is to do a power cycle to clear the cache. As I'm sure you know, you clear the cache by turning off the console by holding the on the console (not the controller) for at least ten seconds. Then unplug the console from the wall for at least 20 seconds.
When you fire the console back up, check the game hub. Launch the game & do some stealth takedowns as a test. Check the game hub to see if the counters have advanced.
After you do this, tell me if the counter is working. Also, what the game hub says your enemies killed and your takedowns are.

You only need to tap to perform a takedown, and was what I did for all 100 for this achievement. Holding the button is a brutal takedown which is something else altogether. It probably still counts, but why make it difficult for yourself?

This achievement takes a while to get but is very easy. There are two takedown options Lethal and Non Lethal. It doesn't matter which setting you have it on.
For this you need to get 100 stealth takedowns. To do this you will need to be in sneak mode by pressing the , and not be detected by the enemy.
A good way of getting kills is to stay in cover and use your whistle ability by holding Once you've whistled an enemy will usually go to investigate the noise. When the enemy is close enough a icon will appear on the enemy. This indicates that you're close enough to get a stealth takedown.
A god tip to add will be to use the button when entering areas with enemies. This will tell you where the enemies are, this is useful for when inside buildings.

I don't think so. I played with no lethal for a bit, then changed it to lethal. What have you been playing with lethal or non lethal?

Glitch warning! The requirements for this trophy must be completed in a single session. This means no turning OFF the PS4 console.
Stealth takedowns are performed by getting close to an enemy and pressing Circle while they are still unaware of your presence. You will need to do that 100 times (in one session, because of the glitch) to earn this trophy.
