Frustrating, but easy to set up. Start up your game and go into Play Ball by using the right stick. You're going to want to make yourself the home team so that you pitch first. I used the Yankees, but the National League or American League All Stars would also be excellent choices, as well as any team with an ace starter (Mariners with Felix for example). The statistically worst team is the Pirates, so you will want to make them your opposition.
Start the game by throwing fastballs and hitting the first three batters. This shouldn't get you thrown out (I never did in my 15 attempts) so don't worry about that. Then the 4th hitter will come up. Get him out and two other outs without the opponent scoring a run.
Obviously the easiest way to do this is some sort of strikeout / double play, but you're going to need a bit of luck. If you fail just hit start, move up to exit game, and hit rematch, rinse and repeat the process.
