The trophy will not unlock with the 5-Star Fiend. The game must be completed twice - on Fetish earning 4 stars at all levels and on Hardcore earning 5 stars, respectively

To get 4 stars you need to complete the level as quickly as possible and do executions where possible (preferably red ones, but sometimes yellow ones are also possible). If you die and click continue from the checkpoint, then the time will be added up, which means that you will not receive 4 stars. Therefore, if you die or feel that you have lost a lot of time, go to the main menu and from there click continue. Then the time will not be added up.

The time to get a star for time on fetish and hardcore is the same. Therefore, when playing a fetish, I advise you to remember the location of the enemies and try to immediately meet the deadline to make it easier to play on hardcore
