As mentioned in the Roadmap, there are two ways to get this: first, you can either complete the game in Normal mode to unlock Hard and then complete the game again on Hard, or secondly you can simply be invited to someone else's game who has already beaten the game on Hard and have them extract to the Doom fight. Whichever way you do it, once you beat Doom on Hard difficulty, the achievement will unlock.
