How to unlock the Fade Away achievement in Mass Effect 2 - Definitive Guide

This is for successfully completing the loyalty mission for
.

Before you can begin his loyalty mission, you must first recruit him on Omega, and after you complete each mission, stop by the main gun room and talk to him. Eventually he will ask for your help dealing with a Turian named Sidonis. If you don't let
kill Sidonis you will fail the mission and won't get the achievement.

I got the achievement before you ever meet up with Sidonis. Right after talking with Harkin and finding out his whereabouts.

Note: If you don’t let Garrus shoot Sidonis and you don’t have sufficient alignment to rectify the situation, he will not become loyal to you and you will not get the achievement.

(Missable) Talk to Archangel in between missions. He is on the third level of the ship, in the main battery. Loyalty missions will not be available until after the mission on Horizon is completed.

Head back to the Citadel. Travel to Zakara level 26 and enter the warehouse to meet the contact. You’ll be told to take a cab over to another warehouse where you can find out where Sidonis is. Of course, a fight breaks out and you have to go through LOKI mechs, Blue Suns mercenaries, and YMIR mechs to get the information. When you finally reach Harkin, prevent Garrus from shooting him in the leg. Once you do, then you have to keep Sidonis talking long enough for Garrus to line up a shot. Since you have two conversation options, it’s really not a difficult thing to accomplish.

After you complete the mission on Horizon, all the characters in your party will ask you to help you with their personal problem. Just talk to the desired character and take the quest; your secretary informs you of all desires to talk to you, so it’s very difficult to miss.

Garrus will ask you for help. At the end of his quest, do not stop him from killing Sidonis, otherwise he will leave the party and you will not receive the trophy.

The trophy will be obtained regardless of the moral choice: let Garrus kill Sidonis or save him from a hole in his head.

I will only add that at the end of the madness you will have a fight with two Ymirs, and this is a problem. Everything is simple, we retreat, climb up the platform and hide behind cover. the whole point is that the ymirs cannot jump, they will stupidly hammer into cover, as soon as they reload, kill them. In my case there was a reboot and deformation. I didn’t spend heavy weapons on a single ymir during madness.
