There are many ways to obtain this trophy. The easiest one is to catch alive (i.e. shoot with a tranquilizer gun) a poisonous animal (red frog, spider, scorpion, cobra, etc.), take it to your inventory through the Backpack menu (the last caught creature should be at the very bottom of the list ) and throw it directly at the enemy.

I received this trophy on the underground floor (with prison cells) of the laboratory in Graniny Gorki Lab B1 East , because... there you don’t have to shit yourself in fear, sitting in the toilets, but calmly run around the building in a white coat. Upon arrival on the ground floor, SAVE! We anticipate the moment when one guard will be in the room, the second will have his back to us, and take off the robe. If you don't have a poisonous animal, take a tranquilizer gun and shoot the spider on the wall. We select the spider, put it in the inventory, pick it up and throw the spider right at the guard’s back. There is a high probability that after this he will reveal you, even if you have time to change clothes, so after receiving the trophy, if the alarm is raised, we reload the save and calmly play on. This is only relevant if you play without deaths or alarms.
