How to unlock the No Rest for the Wicked achievement in Metal: Hellsinger (Xbox One) - Definitive Guide
Update: With the option activated you can shoot quickly with the weapons that still won't miss, that's why it made it easier for me to complete the storms. For the challenges of ending the characters with the I advise you to use the sword, an attack on the smaller ones already leaves them staggering and then just finish it off.

Any extended advice (other than skill) specifically for DEATH'S EDGE III: Kill 99 enemies with no healing available?
I think I'll get it after a few more tries, but it's tough.
UPDATE: Finally finished Death's Edge III. Took about 8 attempts. Used the Vulcan the whole time. Learned how to quick reload. Kept soaring around to dodge attacks. Had only a sliver of health the last 40 enemies which made the Vulcan even stronger.
Keep trying, you'll get it.

Do the torments need to be conquered or completed?

Torments are unlocked after passing through each level of hell. Each level has 3 torments to complete. I am not sure difficulty matters on the torments I played the first four on goat and the last 3 on lamb difficulty I didn't really notice a difference.
In the accessibility options turn on always on beat.
You do not need to get 3 skulls on each torment however, you do need 3 skulls on the first two tiers of the torment for example giant killer 1 and giant killer 2 you need 3 skulls giant killer 3 you just need 1 skull for it to count.
Each torment start with a certain time. Specific kills add time to the clock. Some torments change your weapon after each kill. Some torments you will lose your weapon as the torment progresses. Some torments you can't regain health.
For slaughter kills you need to damage the enemy until prompt appears over the enemy.
For ultimates you need to fill up the yellow meter on the bottom right of the screen then press to activate it.
Remember to use dash to help avoid damage. That will help with the torments where you can't regen health. Sometimes there is the green crystals on the wall to regen health, but not many.
Enemies spawn in the same places so keep that in mind to help you kill faster to keep your time up.
Unless you are in a torment where you can't regain health remember slaughter kills regain some health for you when executed.
