It's suggested to attempt this achievement on the first few "hells" (levels), as they are easier. Select the easiest difficulty, Lamb.
Hit pause
, tab 
to Accessibility, scroll down to "Rhythm Assist Mode", and check the box for "Always On Beat". Doing so makes the game even easier to unlock achievements, but excludes any score from Leaderboards.
--- Paz is one of your weapons. Press
to equip it. It's a skull in the left hand of your character that shoots fireballs by pulling 
.
--- It's ok if another weapon is unholstered, just don't use it and press (or ) until Paz is equipped.
--- You can use the
attack once the meter is full, as long as Paz is being used.
--- You can click
to finish off an enemy to regain health.
--- You're character can die, simply resurrect.
--- You can use the exploding enviromental crystals to kill enemies.
Pull all you want, out of rhythm, and simply finish the level. Achievement unlocks during the level summary. Congrats.

Good to know there are more techniques that don't negate the achievement.
Skipping enemies to speed up the process is always helpful.
Either way, if you're seeing this during the first time through the game, it's an easy achievement.

You can also dash into frozen enemies to kill them much quicker. On the first level you can just skip the enemies in between the arenas, and use the special attack to one shot the Behemoths.

On lamb difficulty and go to accessibility settings and turn the last option on that helps you with perfect beats everytime.
Now only use the skull (paz) throughout one level and the cheevo is yours. Takes a bit longer but is not that hard.
