Start a run with the Hellhorned as your primary clan. As you play through, check unit drafts (or the initial extra cards on higher Covenants) for any Imp cards. Once you get one, simply play it on any floor and the achievement immediately pops.
If, somehow, you reach level 5 in Hellhorned and unlock The Breaker of Horns without this, start a run as the Shardtail Queen, who replaces the starter Torches with imps.
