To get this achievement, all you need to do is commit suicide. The easiest method would be to get to low health, then run straight into a mine field or your own orbital laser beams.

Yep, you get rewarded for failing in this game. *sigh* Oh well, crashing may not be as obvious as you may think, since running into walls or the ground doesn’t cause you to crash. Also, takedowns don’t count as crashes. The only way (I know of) to crash in this game is to run into exploding objects. Do that once and you’ll unlock this trophy.
