Mystic Fate (EP)

This popped even with me dying in the dungeon.
It will pop after you pick up the first orb

Achievement walkthrough:

This achievement is glitched technically in a good way. I didn’t get the achievement after beating the first dungeon without dying; rather I got it after I died 30+ times in the second dungeon.

So instead this achievement is more along the lines of finish a dungeon WITH dying, not without.
