The door opened I fired a flare and it dissapeared unlocking no achievement..... ........... .............

At the beginning of Chapter 5, “Back To Normal”, you will see a cuttlefish behind some glass next to a body. Simply throw a flare, distracting the cuttlefish and unlocking this achievement.
