This, like many of the other in-game stats-based achievements in 2K12, can be achieved through the altering of in-game sliders.
Simply choose a game - any game (though perhaps one of the 15 classic match-up challenges is a good idea, as your win will go towards unlocking all of these and gaining 3 more achievements), and play as you normally would.
Game needs to be of at least 5 minute quarters as well.
Simply be more than 10 points behind going into the last quarter, and then, after the third quarter is finished, go into the menus and alter the gameplay sliders.
My tip is to simply increase all your "user" offensive sliders to 100, while decreasing your "CPU" offensive sliders to 0.
Return to the game and you'll virtually hit every shot you make, while your opponent won't be able to buy a bucket.
This achievement can be done in conjunction with a number of other stat-based achievements, as well as the achievements for playing a game through with manual substitutions, for holding your opponent's FG% under 40%, and stopping them scoring over the final 2 minutes.
If you plan your game well, you can perhaps unlock 6-8 achievements (including this one) in a game.

The only setting that matters for this achievement is the quarter length. 5 minute quarters are minimum for this achievement.
Other then that, you can turn off all NBA rules, and turn User/Computer Sliders up/down.
Turn the computers sliders up until they are up by 10. Once they are up by 10, turn their sliders down, and hold the ball with all NBA rules off until the 4th quarter. Once you reach the 4th quarter begins, Fire away and take the lead, and win.
