Freestyle Pass is simple. Just get an assist using a freestyle pass.
Now to do a freestyle pass just hold the left trigger and pass the ball using the right stick. So aim your right stick at a player who has a shot. Then just shoot with the player that received the ball with your freestyle pass and score. The achievement will unlock soon as you make the shot.

Hold L2 then push the right analogue stick in the direction of a free player. For it to register as an assist you must go straight to the basket after making the pass.
