I was having problems doing this in several ways, then I finally found a way that works.

First of all, just picking a playbook with an option won't work. You have to have a triple option with a fullback in the lineup. I know that sounds redundant, but some of the pistol offenses don't put the FB in the play.

I used the Georgia Tech playbook. You can go into your settings and use any playbook with any team, so you can use your favorite team and change the playbook just for this game.

There are several triple option plays in the GT book with a delay. Press A, then after the HB goes in motion and the ball is hiked, hold A again to hand off to your FB. Hopefully he'll get into the end zone.

Moving the sliders will help a lot, of course, making it harder for the AI to tack and making your FB a beast. You can do this at any time between weeks by editing your roster.

Use a multiple, pistol, or spread playbook, go to choose a play by play type and pick option, choose a triple option play when you are at around the 5 yard line and hold A which will hand it off to the fullback, score and achievement is yours

When looking at the play make sure the fullback is in the formation, some triple options have two HB's instead of a fullback(thanks The Hokiie Kid)

Use two controllers or adjust your sliders to 100 for the User and 0 for the CPU, and change difficulty to Freshman (found in House Rules under the Main Menu's settings). Have an uneven matchup such as Oklahoma vs. San Jose State.

When you have the ball, get down close to their goalline, but don't score (perhaps after "Taking a Stab Downfield" achievement on a 50 yard+ pass).

Now, near the goalline choose your Play, sorting by Play Type. Choose an Option. then choose Triple Option. As soon as you hike the ball, hold A button to hand it to the Fullback. Charge in for the score. I edited my Fullback to almost all 99 ratings to gain the best advantage.
