To receive this ending, simply complete the main story line of the game once. Difficulty does not matter.
Once the credits have finished rolling, you will be prompted to save your game. Do so. Your save file should have a stamp on it that looks like this achievement tile after you save.
At this point, you can load this file from the main menu and begin working on the other three endings.
Achievement should pop after you finish saving and return to the main menu.

Something to note about ending A, and possibly the other endings as well.
The ending A achievement doesn't pop until you save after the ending, and then press B to go back to the main menu. So if you save, and then just turn off the console, the ending achievement is technically missable.

You will get this ending on your first playthru after beating the Shadowlord. It doesn't matter if you try for the 15 hour achievement or not, this will always be the ending you get.
It will pop after 6 hours of credits. (ok, not really 6 hours, but the credits are forced, you can't skip em and it feels like forever waiting for that achievement !)
Difficulty doesn't matter. I played on easy.

The secret to passing through the credits of ANY game quickly... (This secret only works if you also have a PS3 and a Netflix account!) Change the channel, watch something on Netflix. :P

After completing the game for the first time you will be presented with Ending A. Once the cutscene has ended you will be prompted to enter the name you called your character, not Dad as some assume, it's also case sensitive. After the unskippable credits the trophy will unlock.
