Enter the BOY house, and once inside hold left on L3 and press X to choose to quit the game, then hold right and press X again to confirm that you want to quit. Bye-Bye BOY will come out of the chimney and the trophy will unlock.
Note: You will have to quit to the XMB and load the game back up if you wish to continue playing.

When inside Boy's house go to Quit Game and Fairy will come out and then Bye-Bye Boy will come out of the chimney. Congratulations now you have to go back to XMB and restart the game if you want to keep playing.
