This is the first boss, he’s a giant surgeon (duh) in a plague mask wielding a giant pair of scissors. All enemies in the game have AI rules and they don’t deviate. You can check them in your journal thing between missions but basically here’s what happens:
He spawns with several plague dudes (Caretakers), these guys will shield him making him pretty unkillable so you have to kill them first. They’ll only attack your Daughters if they activate directly adjacent to one, so don’t end an activation in melee with one.
Big Jerky-McJerk-Face isn’t too aggressive while his plague minions are around. His AI targets the closest Daughter during this phase and he can summon new Caretakers so be aware. Just make sure you have an interrupt ability active to protect whoever is closest to him as he hits obscenely hard.
After you blast down his final minion he goes nuts. His AI changes to attacking the Daughter with the most life. You can keep this consistent so long as you never let him land a hit on her and get Flank or Backstab bonuses.
He is really, really fast during this phase and buffs himself to move up on the action bar every time he activates. He also moves very far, and charges up a large AOE (luckily it’s slow).
I recommend 1 Shield Bearer and 2 Soulslingers for this fight. You need the Soulslingers to have a good health pool because you’re going to be burning it down. Use Interrupting Round to stop his attacks. Make sure you have good Memories attached to their skills before entering the fight.
Be ready, he immediately attacks after the final Caretaker goes down and if you let him jump from Daughter to Daughter it gets out of hand fast.
If you have Shadow Round on your Soulslingers also put that up every round it really does add a ton of damage (don’t worry, you can definitely kill him if you didn’t choose Shadow Round, but after this Germinate new Soulslingers with the plan to give them Shadow Round and work on them instead). You want your Shield Bearer to be the target (sacrifice a level 1 Shield Bearer Daughter to her as soon as you start your run for extra HP).
Use Slam on your Shield Bearer to force him to face away from your Soulslingers, then move out of his AOE. If you have Blacksmith’s Grace you can use it just in case but be careful you don’t burn your HP below your Soulslingers.
Don’t go into Burst if you can help it, until you’re on the home stretch it’s a good way to wipe. Maybe on one Daughter to take down the final Caretakers at the right time but that’s it.
