DON'T DIE! ... *ahem* sorry, but yes, to get this achievement, you must not die during Score Attack (5 Minutes). Use bombs as needed and if you find it difficult, start the level on Easy setting. That will keep the speed low for nearly half of the time on the level, which should allow less changes to die.
