I had a couple attempts trying to unlock this, I finally did it and this is how I did it.
-Turn on god mode and rapid fire. Follow Cheevo guides video on how to input them.
-Don’t bother shooting enemies, only shoot the bosses
-ONLY TAP THE A BUTTON, DO NOT HOLD IT DOWN
I think the issue where most people are not unlocking this if they are following the guide is that normally when you hold down the B button it locks on to a target as sort of an auto aim mechanic. When you put in the rapid fire cheat it doesn’t lock on to any enemies, but the game probably thinks you’re trying to auto aim so that’s might be the reason it’s not unlocking on the first play through.

I got it first try by playing through on easy (first playthrough) and focusing on avoiding shots and just tapping A the whole time. Accidentally held A once and was worried that would void it, but it didn't lock on to anything and the achievement popped after the final boss.

I will also add that it is worth changing the control layout to 'Modern' rather then 'Classic'.
This is because LT becomes fire and RT becomes lock-on fire. So you don't ever have to worry about accidentally holding down LT and it locking on - it never will. I tested this in game and it never locked, so this achievement popped as planned at the end of the game.
