Chapter 5 - Acceptance
The Achievement is a bit wrong descripted in my oppinion you dont actually see anything i just think the Game will add 1 Token towards this Achievement if you get the other 4 Achievements on the Chapter where Ewa ask you about the final Decisions.
So you need these 4 Achievements to get the Achievement for all Endings
I Can't Do It- Don’t unplug Ewa
Sleep, Sister - Unplug Ewa
Don't Look Back - Leave the bunker
Beacon of Hope - Stay in the bunker
so we cover all 4 Achievements in 1 Playthrough also we get the Achievement for seing all Endings and we complete the Chapter 5.
But i explain everything in my Walkthrough.
Its very important that you do exactly the same as me in my 100% Walkthrough otherwise you have to replay the whole Game.

here is my Guide for 1 of the missable Achievements we need
On the last Chapter 5 - Acceptance
Is where we going to decide about the Future/Game Endings and final Decisions.
You will come to a point shortly after starting the Chapter where you see Ewa in a Maschine and she will be asking us a few Questions.
She ask us if we Unplug her or not and each Decision will get you 1 seperate Achievement.
Afterwards sher will ask us if we stay in the Bunker or we leave the Bunker.
So thats 4 Achievements in Total and also we need all 4 of them to get the Achievement for Putting the Pieces Together - See all the endings.
You can press Pause as soon you get the Achievement or after you decide to stay in Bunker or not and load Checkpoint and go for the other Decisions.
For Example
1st Time:
Unplug Ewa and Leave the Bunker( as soon the Achievement pops for Leaving Bunker press start and load Checkpoint)
2 Time:
Dont Unplug Ewa and stay in the Bunker

So uh, what about unplugging and Not leaving? Or not unplugging And leaving? Are those endings or are we consigned to making the two connected choices for an ending?

just be sure you reload your Checkpoint after you unlock the second Achievement and after this just go for the Different Choices...with all 4 you get the ending achievement

I show all endings in one playthrough in my Achievement Walkthrough with Commentary using a very easy load last checkpoint method.
Choices to make at the end of Chapter 5: Acceptance.
Unplug Ewa, Stay in the Bunker, QUICKLY reload last checkpoint after getting the two Achievements.
This time choose, Don't Unplug Ewa, Leave the bunker, and let the game end and the achievement for seeing all endings will unlock during the final cutscene/credits.

There are 4 ending-related trophies: I Can't Do It
, Sleep, Sister , Don't Look Back 
and Beacon of Hope . You can get one Eva and one bunker trophy at a time but there is a way to get them all in the same playthrough so you don't need an entire second run to get the two you didn't earn on your first run.
At the very end of Chapter 5, Acceptance, you'll have a small talk with Eva and will need to make two decisions. The 1st one will be to decide her fate and the 2nd will be to decide yours. You can choose either or for each but to make it a smidgen easier on yourself, just choose the top two options first. This will give you Sleep, Sister and Beacon of Hope . Let the ending cutscenes play out and it'll eventually cut to a short cutscene just outside of a snowy bunker:
As soon as this cutscene starts, pause the game and choose "reload checkpoint". This will start you back at the beginning of the final scene with Ewa. Go through the dialogue again and this time choose the second option for both decisions. This will get you I Can't Do It and Don't Look Back . Let the ending cutscenes play out and when you reach the cutscene with the bunker, the trophy will unlock. If you do not already have Acceptance , let this cutscene with the bunker play out until it unlocks. If you exit out too early and start a new game, you'll miss it and will need to play a complete second playthrough to get it. If you prefer to use a video walkthrough, please see below:
