This is very simple to do -
During Chapter 1: The Wardstone Legacy you will be given a sub quest 'Spies amidst our ranks'. Which will require you scout out the Tower of Estrod location. It will be marked on your map during story progress.
In this location will be a large and tough group of enemies, on the right side of them will be a broken pillar with an Athletics 13 skill check. I played on core and was able to rush across to the pillar stand in front of it and use the skill check. As long as at least one of your party gets crushed to death you will pop this achievement.
Notes - Make sure to make a save so you can reload the damage.
