Phantom Abyss

At the beginning of the game you're basically in a tutorial section you are not in a temple. Once you finish the tutorial section you can actually enter a temple and you'll get a loading screen that shows the different floors of the temple. Once this Temple begins you have three hearts of life and when you lose all three hearts you get your first death. The achievement will unlock once it tallies your score for that temple.
