There are 24 regions and all have to be explored as part of the story progression the achievement will unlock when you enter the temple to fight the final boss.

There are 24 levels, each has one corrupted plate that you will need to heal. To finish the game you will need to heal all 24 corrupted lands. The achievement will unlock after reaching the temple for the final boss battle.

This achievement is part of the storyline and CANNOT be missed.

Story related and unmissable.

The very last region is not actually on your map so don't stress when it doesn't unlock after you heal the final fertile ground, it will unlock after you enter the Temple to face off with the final boss and will be the first trophy you unlock after installing Patch 1.01.
