i would also like to add that if you have finished the game befor hand on a easyer difficultey then all the reversals you did will also carry over from your other game

I used the underground fights in chapter 1 on Shark difficulty.
When a fighter - in my case Soda - starts to throw a punch, hit right bumper (RB). If you have timed it right you will get a button prompt - A, X or Y - I never saw B come up.
I did the whole 50 using Soda on Shark difficulty. I got 3 or 4 reversals per fight so it didn't take me that many fights to complete this achievement.
At the time I was working on the "Mogul" + "Doesn't Play Well With Others" achievements.
