This is a story related cheevo and cannot be missed.
You will get this at the end of chapter 8.
Just after you get the "GUARDIAN ANGEL" cheevo you will be running through some sewers again. at the end of the sewers is bellick lying on the floor just starting to get up. run up to him and start the action sequence.
Press the correct buttons and beat him down. once thats done you will climb the ladder and cheevo should pop
