XBOX FOR PC PLAYERS:
Hit the ~ key and enter "map e3m4" (please note level select seems to nullify some achievements, but using the command code work with no problems)
Now enter the usually "god" and "noclip"
You will want to fly to the bottom of the level. The portal to the secret level is that portal, the one below everything else. You will need to turn "NOCLIP" off before entering the portal. Once you enter the achievement will pop.

People downvoting your guides just because they're geared toward PC need to mellow out and connect a controller to their PC. I have one with a cord I got with my Xbox One X NBA 2k20 bundle that I keep connected to my PC. It's not your fault the Xbox One version has no keyboard & mouse support.

Yeah, thanks for the support. Of course, I agree. But there is not much I can do about grumpy granny gammers and entitled Debby Downers. I still have a positive ratio of upvotes and the guides are helping out some people so... I don't mind too much.

The secret exit for Episode 3 is found in E3M4: Satan's Dark Delight. This is a written guide + images to find the secret exit, a video clip is also provided at the bottom if you prefer.
Towards the end of the level you will have to ride a bunch of moving platforms, after which is an elevator. At the top of the elevator is a room with 8 lights above you - notice that one is yellow. Shoot the yellow light and a secret will be revealed, you'll want to use the damage protection powerup here in a moment.
Take the powerup then do a 180 and go down the hall. At the bottom of the stairs will be the Thunderbolt weapon, but when you approach a Shambler will spawn on it. With the protection powerup you can just let him attack you and take him out as you see fit. When it's dead, go down the stairs and again pull a 180.
Underneath the stairs in some lava and a large hole in the middle farther down. Make a jump into that hole and the wind tunnel will take you directly to the secret exit.

Here ill be putting steps for the achievements as well:
1) choose e3m4 satans dark delight on easy
2) take the left path into the water and follow it to the next door
3) meele everything but the ogres fire 8 shots then melee them to conserve ammo it take 9 shotgun shots to kill an orgre
4) up the elevator and around the corner theres a switch to hit to continue
5) go straight past the rocket launcher to continue into the room with the switch and hit it
6) turn around and shoot the yellow light theres ammo and health if you need it
7) continue following the path to the end and hit the switch follow the path till you drop down
8) get on the moving pathform and ride it to the end shoot the switch you see then enter the crusher
9)get on the second moving platform and shoot the floor switch follow the path of platforms to the elevator
10) clear the top of the elevator follow the path you will see a lightning gun that will spawn a shambler deal with it the turn around the secret exit is under the stairs where the lava is save here just in case you fall in
