On E1M5: GLOOM KEEP (Easy), going into the large building in front of you when you start you'll eventually come to a choice of going left or right.
Go right and clear out the area ahead of enemies until you reach a portal. You can enter the portal from the side you first see to go to where the Shambler is, or the other side of the portal to go to an area on top of the building you entered.
On top of the building you'll find Quad Damage. I would recommend playing through other parts of the level to stock up on armor and health but there is some health where you will fight the Shambler.
Grab the Quad Damage and (as a commenter (Juju) mentions) head back through the portal on top of the building and you'll be taken to the Shambler. With the Quad Damage and keeping your health up, you should be able to Axe the Shambler to death after not too long.

I'll add the same note here that I've added to the Shambler Dance achievement: When you enter the portal from the backside, you'll come out of a portal at the top of the keep. Clear the area, grab the Quad Damage and head back through this portal and you'll be taken to the Shambler.
You can also do this on Nightmare. When you start the level, if you jump off of the bridge and head underwater, you'll find a secret cave along the right hand side wall, opposite to the keep. In this cave you'll find one of those life boxes that give you over 100HP. I recommend saving it until it's time to face the Shambler.
When you get through the portal, there'll be an Ogre behind you when you go up to fight the Shambler, but you can run around the Shambler and have the Ogre deal extra damage to it while you axe away.
Also, I got this achievement yesterday without killing the Shambler on Nightmare. I was attacking and mistimed my evasions, taking a hit and dying. The achievement popped while the Shambler started roaming the room, so maybe you don't really have to kill it, just deal enough damage.

Thanks I'll change the details to reflect going back through.
I had the same experience with it popping before killing the Shambler, finished it off with the axe on principal.

Map: E5END: The Year Zero
Difficulty: Easy
In the beginning turn 180 degrees around and go to the underwater tunnel under the red portal. Turn left and get the yellow armour. Then continue to shambler, who waits where you were heading at the start of the level. There's Quad Damage underwater on the opposite side of stairs which lead to Shambler. Takes about 5-6 strikes and with the armour it's like a walk in the park.

hi this is my first achievement guide video i hope it helps
Here ill be putting steps for the achievements as well:
1: Load level E2M2 the Ogre Citadel normal/medium difficulty (i recommend just playing E2M1 first to get some ammo)
2: After you clear the room with the 4 knights in the make a right and go down the stairs and follow the path to the nail gun and quad damage save before picking up both
3: jump the gap grab any health kits you need and drop down into the room you can get both achievements in one run by meleeing and killing the shambler before it casts lightning but if you dont get it doing it that way then just reload the save and use the nail gun with quad damage to make very quick work of it
