XBOX FOR PC PLAYERS:
Hit the ~ key and enter "map e4m5" (or the level select method).
Now enter the usually "god" and "noclip".
Fly around until you see either a big exit portal or an exit arch. They are next to each other. You want the portal, not the arch. The "secret" exit is right next to the main arch exist behind a silver key door. Fly through that door, turn off noclip and jump through the portal to unlock the achievement.
(please note level select seems to nullify some achievements, but using the command code work with no problems)

None that I have read about yet. I am still making guides for everything. Of course, if I find any info I will post asap. They have level select on console... so surely the cheats are hidden in there somewhere too.

The secret exit for Episode 4 is found in E4M5: Hell's Atrium. This is a written guide + images to find the secret exit, a video clip is also provided at the bottom if you prefer - I decided to to a full speedrun of the level because of the complexity of this secret.
This level has an interesting quirk - you can actually reach the normal level exit very quickly by jumping on the thin poles of a bridge you have to lower. For the secret exit however, you must collect the gold and then silver keys, and make sure not to lower that bridge or it will be locked off.
Around the first corner of the level will be a wide open area. To the right is an area with the gold key, to the left is an area with the silver key, and ahead is the area with the aforementioned bridge. The first step is to fight your way and obtain the gold key, watch out as enemies will spawn when you grab it.
Now cross over and open the gold key door. Go forward and take a left, this area is filled with zombies. In this area you will find two rooms side by side with buttons, the one on the right has a damage protection powerup you can grab. Hitting the button on the left will drop you down below.
Now this area is large and filled with dangerous enemies. There's a quad damage at the start so use that and the damage protection to clear out as much as you can. You'll want to head right then follow the corridor as it loops back around, take another right when you can into an area with stained glass windows.
Navigate this area to find another button on the ground. Hitting this will make all the windows open and spawn more enemies in. Behind one of the windows with a zombie near the button is another Quad damage. Grab that and double back through one of the newly open windows, in this area is a well hidden ramp you can climb and finally grab the silver key.
Now exit the stained glass area back into the corridors and hang a right. At the end on the right is a portal, when you approach it will move back and more enemies spawn. There are buttons on the left and right to extend a bridge but you can also just hold jump and skip across to the portal.
Finally make your way back to the open area and towards the central passage with the bridge. A few more enemies and powerups in here so take them all out. Now you just have to make the tricky jump across the bridge poles, quick save/load as needed until you get across. Head through the silver key door on the left to find the secret exit.

TL;DR Play through the level normally till the very end, at which point jump on the bridge poles to make it to the secret exit.

hi im back with another achievement guide video i hope you like it
here's the written steps:
1) use the level select to select e4m5 hells atrium on easy difficulty
2) head to main room take a right and pick up the gold key thats in the room there
3) now that you have the gold key head back to the main room and head across the way you will find a golden key door
4) head left here into a room full of zombies head right and you will see a gate with a power up behind hit the switch on the floor hop over and hit the switch on the other side
5) now head right and thit the switch on the wall drop down and take the right path
6) go left here pick up the lightning gun then head into the room full of stain glass find the room with the switch on the floor and hit it
7) now back track one of the stain glass walls will open up take the right path and follow it up to the silver key
8)back track to the start of the stain glass room where you got the lighting gun and head right down the hall youll see a portal chase it down and enter it
9) now head back to the main room of the level and head left you will see a ring of invisibility clear the enemies and head towards the draw bridge
10) do not hit the silver key switch to lower the draw bridge you will be locked out of the secret exit instead save here jump across the 2 wooden poles above the lave once on the other side enter the secret exit
