For PC gamers:
This one was buggy for me. So, maybe try a couple times or give it a minute or to pop. "good Super man" helped me with this solution so props to him for helping me trouble shoot this one. He also found a video I am going to link that was a big help.
From the main menu:
1. select "single player"
2. select "new game"
3. select "Dissolution of Eternity"
4. You need to access nightmare. First run through normal portal. turnaround and there is a portal to take you back. There is a switch behind that portal. Shoot through that portal. On the left and right above you and around the each corner are two more switches. These will turn on a lift in the middle. It's really hard to explain in text. If you watch this video at this time it makes it much easier to understand:
4. hit ~ to bring up console command
5. type "map r2m8" (you have to start THIS campaign to be able to warp this level).
6. type "god" and "noclip" and fly to a room with a round thing in the middle. The teleporter is in that but you need to do some stuff to end this level. You will also see a big room with a claw and lighting. I think you need to open that room to activate the exit for the level. So let's do that: back in the room with the round ball thing, there are 4 switches. Simply hit all four of those and the claw with lighting room will open. Kill the claw device and the level will end. Done!

5. Once you jump through the nightmare portal then jump through the episode portal on the right side (I am not 100% sure if this is needed, but like I said I was having trouble with this one popping and this is how I got it)

I got all the difficulty specific achievements on my first try, huge thanks to GR34TD3STR0Y3R for the awesome guides! Here are the steps I took, hopefully this will help out some gamers that are having trouble getting it to pop.
First off, I never typed the difficulty into the console. I always selected the difficulty from inside the game, such as going through the water in the base game. Then I would always load into the first level normally. From here I would start using the console. I would type in the map first. Once I loaded into the map, I would pause the game real fast because sometimes enemies will be close by. Then I typed "sv_cheats 1" and then "god" into the console. Depending on the map I would either kill a boss or type "noclip" into the console and just fly to the end portal. Before I beat the level, I would always type in "sv_cheats 0" to turn off all cheats.
I also wanted to clarify that this is a play anywhere game. So if you bought it on an Xbox One or Series X|S, you can play it on a PC. It will show up as the same game in your library and will have one unified list of achievements. I can 100% confirm this because I did this myself. You just need to have the Xbox app (and all the system requirements like Windows 10 etc.). You can sign into the Xbox app and simply download the game to your PC, it's only 1 gb. The game will sync perfectly, all your saves will be there and even your settings will be the same.

skill 3, map r2m8, god, noclip, fly to platform next to claw, noclip, shoot claw, achievement unlocked.
