EDIT#2: This achievement along with at least one other may have been broken by the most recent update. I will update as soon as it seems it's working again.
hit the ~ key and type "map e1m7" and finish the level. You need to pick up the rune in front of you. Turn on "god" mode (console command). As long as you pick up the rune, you can turn on "noclip" and go straight to the portal this time, no need to kill the boss. Jump through the portal for your achievement.
(please note level select seems to nullify some achievements, but using the command code work with no problems)
***EDIT: It seems GOD and NOCLIP have been disabled in a patch***

According to a recent post on reddit you need to type "sv_cheats 1" hit enter and then type "god". You should get a notif in the console that god mode is on.
MAKE SURE YOU TYPE "sv_cheats 0" AFTER YOU HAVE ACTIVATED GOD MODE OTHERWISE YOU WILL NULL THE ACHIEVEMENT. God mode will still stay active after entering

XtremeTIZZYactually this site is for "achievements" and the website wouldn't be doing its job if it also didn't include the achievement's that were on all Microsoft platforms, including pc,
as stated this game does not have an individual windows 10 stack and shares its stack with the Xbox version. its clear that it belongs here, just because it doesn't help you specifically does not mean it is not helpful to hundreds of other people who use this site.
one of the paragraphs laid out in the "achievement solution guidelines" for posting solutions for achievements states this information, I have highlighted the portion that directly relates as to why this is a valid solution laid out by the site rules and procedures.

I got nightmare achievements to work on all the campaigns. I simply started a new game on all the campaigns found where the nightmare portal was entered it to the 1st level than just skipped to the finale and unlocked the achievements. I found a video which shows every nightmare portal.

Tried this a few times on pc and the achievement didn't unlock. I made sure I picked up the rune and tried both no clipping towards the portal and did the level normally and the achievement never popped for me. Idk if I missing something else.

Did you use level select or enter the map code through the Comand Line.
I found out you can NOT use level select. I am amending all my guides to point that out. You have to either get their naturally or enter the map code.

I did console command and level select. Just found a way to do it. I started a new game went to do the nightmare portal for the other achievements then started the 1st level of episode one then did the e1m7 console command and it unlocked after beating it.

I just had one of the other added campaigns glitch on me. I can get the nightmare stuff to work on one, but not the other. I am doing it the same.

I think for campaign-related achievements hit new game then go through the portal of what you need and enter the 1st level then do the console command and beat it.

I think that the game isn't counting progress if you try to use the console command in the menu or in the hub world choosing which episode to play.

So far the only problem I'm having now is beat the game on nightmare and I tried using the level select console command and no clipped to where the level would be after using the nightmare portal and neither are working for me.

nvm it did unlock it was just a bug where it didn't show me unlocking it and showed it was locked til I exited out of the game.

Edit: ahh... gotcha. I am having unlocking night mare on 2 of the 3 new campaigns.
1 works like it should. The other 2 are not unlocking.

@tajbender Sometime when come to WIN10 and XBOX version the game. They share the same list. So it be hard for TA to make two list. Other PC list. Is the STEAM version. And not XBOX PC version. When XBOX console and XBOX PC has their owned list. One be listed as WIN 10 while other be listed with no platform on it.

None of the Episode achievements have been unlocking for me in Quake on Xbox One, but the other ones have (Shambler etc). I'm playing through each episode, level by level. I did Episode 3 last night in a full session run without any luck.

I definitely had trouble with the last campaign unlocking for me. I did a few times. It finally popped, but I never figured out why it popped or why it didn't.
Try shutting your console all the way day and back on again. I know for some games that is certainly a problem. I had to do that for about 8 Watch Dog Legion achievements to pop.

It didn‘t work for me either. I remembered that I started the episode from level select so I started it again with the new game option in the main menu. That did the trick. I don‘t know if that was a unique situation for me but maybe you could try that.

There is another achievement that is broken because of the new update. People have put in support tickets and Bestheda does not seem keen on acknowledging its an issue.

To BabyishDuck if you haven't figured it out yet, also on a UK keyboard here and tilde doesn't work. For me the top left key on my keyboard which looks like ¬ and ` is what works to bring up the console.

I can't remember exactly, but someone posted in another solution of mine that I think if it's a UK keyboard it's the ¬ key.
No matter what region keyboard you have you should have a button that will open the command console. Comand consoles have been around in games since the dawn of DOS.

Can't get God mode to work but guide still works. Only thing I would add is if you use noclip you need to turn it off when you get to the portal by using sv_cheats 0 and then walk through the portal

Also, it seems like for achievements like find the secret portal and find the well I had to start a new game and enter those episodes first before using the console command to teleport there so I could unlock them.

This solution is for the "Microsoft Windows 10 version" of the game, which shares its achievement stack with the Xbox console version.
They are one in the same and this solution all be it more aimed at pc ultimate game pass players is 100% valid, and thus belongs here.
As the game clearly states :
"Xbox One, Xbox Series X|S, Windows (Windows 10)"
and
"xCloud, Xbox Game Pass for PC, Xbox Game Pass"
when the quake game gets its own achievement stack "quake (windows 10)" then at that point it can be moved. however that doesn't exist so here we are.

Dudes, I figured out why this is glitching. Super simple solution. Make sure to actually start a New Game on "Quake" BEFORE you type in the command for the level. Other wise the game thinks you are using the level select. Works every time this way as of May 22.
