The Basilisk is located on a road between Hilvalla and Tjolmar. Before getting there, you need to make sure you have a brass mirror. You can find them on enemies or purchase one from general good stores. Head to Tjolmar if you need to buy one. The screenshot below shows the road it's located on.
Be sure to save BEFORE reaching the end of the road.
Once you're there, you will need to pick one of your party members to sneak up on the Basilisk. This party member MUST have high skills in stealth, dexterity, courage, and agility. You should at least be in the range of 15-20 for all of these skills, otherwise, you won't survive. They also MUST have the mirror. So when you are asked to take an item with you, bring it along.
Keep approaching the Basilisk. Your goal is to not be seen, at least not until your 10 paces away. If you are caught, it will petrify your party member and ruin the attempt. Once you're 2 steps away, use the mirror. If you succeed, the achievement will pop! Check back later for a reward.
