If you whistle for your horse and it runs off a nearby cliff, don't follow.
Seriously though...Play with a couple teammates; being incapacitated while playing alone will cause you to lose this achievement and restart the mission. Take your time, and stick with your posse...Unless enemies are throwing TNT. Lastly, take cover frequently if you're that much of a bullet-magnet.
Simple achievement, just don't stress out...or follow your horse too closely.

LMAO! My horse was falling off cliffs committing suicide in single player a few times when I whistled for him. Shit had be on the floor! I thought it was just me! =D
You just can't make up hilarious things like this up. Only Rockstar can pull off unintentional things like this with there damn glitches and bugs. Too funny!

So this guide is a walkthrough of the level the escape telling you exactly how its done!
Hey all, this is my first foray into posting guides and I've chosen to show you guys a guide to the first 5 achievements of the Co-Op these are so this applies to 5 different parts of this site but the video covers it all! Just read the description:
• Stake a claim: gold medal any co-op mission
• Well done complete a co-op mission
• Bullet proof complete a co-op mission without dying
• 2 guys 1 co-op 1 mission
• Friends indeed complete a co-op mission without anyone dying
There are extensive notes and a commentary on the video link and it is definite walk through of the first level so if you want to experience it yourself I'd suggest coming back to it!

Once you start one of the missions, take cover with the 
and take your time. If you rush into the middle of a gun fight you won't last very long. You can also keep your distance and pick enemies off with a rifle.
You can choose your load out at the start of each mission by running up to one of the tailors dress mannequins and hit 
to change your load out.
Make it though one of the missions without dying and you will be rewarded for staying alive.
