I did this in the first level. Just aim for headshots and don't waste any bullets. It is ok if you take damage. This video shows my 100% accuracy run on the first level.

The trophy is completed in a similar way to “Hats Off” (6 headshots from a revolver). After completing the game for the first time, the “Bounty Hunter” mode will open, which allows you to choose any mission. Launch the first “Bull's Eye”, skip the training and clear the level of bandits without missing a single shot. Try to shoot in the heads.
