Story-related. Cannot be missed.

Straight after the Ultor Personnel Suppressor, you will end up in the Medical Research Facility. You can stealth here if you like but it's not worth losing your weapons just to skip a few enemies so I recommend going loud. After you get the keycard from the Medical Administrator and continue into the restricted area, you will meet Capek and get the trophy.
