Need a defense item for this to work and it is a objective for you to do during the Andrea "boss" fight, not miss-able at all.
Have the stalker walk over to a area by either, calling out to them by pressing the Left D-pad, or by throwing a Diversion item to the area you want them to go to.
(Note that sometimes this may not work so please be patient on repeating this process if it fails the first time)
After the stalker walks to the area and they don't see you sneak up to them and you will see the A button command on the top of there head, press A and you will unlock the achievement.

For this, while equipped with a defensive item, throw a distraction item like a bottle/cup or place a music box/toy doll. Allow the enemy to inspect it, then follow either crouching or running up to the enemy, and pressing
