How to unlock the Written Word is Dead achievement in Resident Evil - Definitive Guide

The Achievement can be obtained after you have killed Plant 42 at the Residence Building and returned to the Mansion with the Helmet Key.

In order to obtain the "Last Book Vol.2" you will have to make your way to the Library (Mansion Map 2F West Side).
Start from the Medical Storage Save Room (Mansion Map 1F West Side) and make you way upstairs. Run through the area where you found the Arrow, past the Crimson Head Zombie and through the door that leads upstairs to the Knights Armour Puzzle.
Run to the end of the narrow corridor and unlock the door (Helmet Key) leading to the upper section of the Library and follow it to the end.

BOSS FIGHT: YAWN (Round 2)

Easy way to kill Yawn is to use your Combat Shotgun and keep running in circles around the bookshelf. Run, stop, shoot, run stop, shoot... should do the job nice and easy.
After you have killed Yawn you can go behind the bookshelf and pick up the book.

Just a fix for the buggy- portion of this.

The only way it will unlock is to delete all your save files from both the console and the cloud before getting this again. If it bugs for you, don't delete your saves until you have done the rest of the achievements otherwise you would lose progress for the outfits etc.

This is a Chris only storyline trophy. To earn this trophy you must defeat Yawn inside the Mansion Library as Chris; access to the Library is earned via the Helmet Key. Once the boss is dead you’ll be able to pick up the book and earn this trophy.

In order to get the Last Book Vol. 2, you need the Helmet Key (which is obtained after defeating Plant 42). Go back to the Mansion and make your way to the Armor Puzzle in Death's Corridor (where you got the Armor Key) and run passed that to a locked door at the end. Head through and you'll eventually encounter Yawn. You need to defeat the Giant Snake here for it to knock the book loose, grab it and the achievement will unlock.

The easiest way to defeat Yawn as Chris is to lure it behind the bookshelf where the Last Book is kept and fire at it a few times, before heading into the main library. Circle around, do the same thing over and over. You will also use this method on your knife run but you'll be stabbing the back of the snake's tail. Use the assault shotgun if you obtained it from Richard in the Aqua Ring or the regular shotgun.
