The first level that you can earn this on is Ngari which is the second level. A Story Altar is visually almost identical to a Save Altar. The difference is that the Story Altar has a completely open book on top whereas a Save Altar has what looks like a scroll that is only partially open. The Story Altar is also brighter.

The Story Altar in the Ngari level can easily be missed as it is located off the beaten path in the second area. You are required to collect Tukus for the Collector: Ngari trophy and if you are following the Tuku guide then you pretty much cannot miss this Story Altar as it is literally right next to the Tuku in the Market Square (second area of the Ngari level). The trophy will unlock shortly after you press in front of this Story Altar but you can unlock this trophy in any level that has a Story Altar.
