You can access a shop in between missions. You buy upgrades with "radiation coins" obtained through both killing enemies and cleaning up radiation with your water gun.
The medkit upgrade, letting you start each mission with 2 medkits (as opposed to 1 that you begin with) costs 200 rad coins.
A fast way to earn coins is to look for any clearing that has multiple piles of garbage, all surrounded with those green sparkles that you are shown in the tutorial that need cleaning up. You'll know radiation is around when your rad counter begins going up.
This upgrade can easily be purchased after mission one if you are diligent enough with your water cannon.
