How to unlock the Frag Out! achievement in Rogue Warrior - Definitive Guide

You should get this as you progress through your 3 playthroughs. Just keep throwing grenades into groups of soldiers. They like to run away, so multiple grenades in an area is a good idea.

If you haven't accomplished this by your 3rd playthrough load up Level 1 and when you get to the courtyard, throw a grenade at the door and you should get a couple grenade kills each time. Load up your checkpoint and you will be in the same courtyard so just repeat and you should have 20 pretty quickly.

Don't be shy about using your grenades. In fact, often times the cruddy AI will just stand by the grenade that you lob, or they'll even run into it. You should get this one about halfway through if you're not saving them for a rainy day.

You should get this way before the end of your first playthrough if you use them frequently. Another quick way is once you start to exit the courtyard on the first level, four enemies should burst through the door. If you can time it right you can get all four with grenade. Once you have done this press start and reload the last checkpoint. Keep repeating this until you earn the achievement.
