How to unlock the Ouch. achievement in Saints Row: The Third Remastered - Definitive Guide
1. Ignore the "zones".
2. Once activity commences, grab a car, and drive to a highway.
3. If you can find an exit of a highway with a down ramp, all the more better. If not, it's not crucial. It's just that the speed of cars would be faster, potentially giving you more points.
4. There's an unlimited supply of cars on the highway, and they usually come in batches, making it easier for you to chain the hits.
5. Important: do not let the cars hit you - instead, time it right, and ragdoll (LT/RT) right before the car hits you. This sends you flying in the air - more points, better chance of hitting another car for a chain.
6. You earn adrenaline as you progress. Make use of it to score more points by chaining more cars as you fly - it's easier said than done but with the large number of cars, even hitting a stationery car would give you more points.
Glitch alert (good one): if you happen to be in adrenaline as the clock reaches zero, the activity will continue until adrenaline depletes, essentially giving you free time to net more points. That said, if you're on a highway, hitting the required points (70,000 for Easy, 200,000 for Medium and 450,000 for Hard) shouldn't be an issue. I did all of them with several minutes to spare.

Head for the next big street, run and jump straight into cars. While in air, try to hit the following cars.

Icon: Crutches (If the icon is grayish, it means you’ve completed that instance.)
You’ll act like a human ragdoll. By pressing either or , you’ll drop to the ground. You’ll earn money by doing this. Get the money required before the time runs out. The fastest way is by throwing yourself at oncoming traffic. Use your analog stick to move around when hit, to chain together multiple hits from the same, or multiple cars. Move downwards, so that you’ll keep hitting the same car or multiple cars. Try to get your adrenaline bar full so you can chain together some pretty big hits. You can almost freely fly yourself. If you move to one of the blue targets, a circle will appear on your minimap. Stay in that circle to gain adrenaline faster. The circle will disappear after a while.
See the ‘Complete all City Takeover’ trophies for all the activities per district. If you can’t find an activity, you either need to complete more of the same activity, or check the map for a blue icon with a ‘?’, which is a hidden activity.

For this activity you have to rack up a certain amount of points by throwing yourself into oncoming vehicles. Push or when you are about to be hit to activate a rag-doll like mode. You will often get launched up into the air so try to aim yourself to bounce off of another vehicle. Your point counter will keep going up as long as you don't touch the ground.
Tips:
Try running at a vehicle head on, push to roll, and then push mid roll. This techniques launches you at the car at a faster speed and gives you more points.
If you are close to the total points that are needed, but the time is running out, you can repeatedly hit to drop to the floor and get anywhere between 300-600 points for every drop. Even if time as up, the activity doesn't end as long as you're scoring points.
You should also try driving at a vehicle head on while in a light vehicle. You will likely get ejected from the vehicle, scoring good points while also working on the Windshield Cannon challenge.
