The maximum upgrade level for any piece of equipment is 7. The trophy requirement says you must upgrade a weapon to its maximum level, even though you may also upgrade your shield or your armor to its maximum level.

A weapon is upgradeable at blacksmiths. You must offer a Stone Blacksmith to a sanctuary your creed owns and select the upgrade option. To upgrade a piece of equipment you must have its related upgrade material and the amount of salt required to it. There are five groups of upgrades sequence. You will quite easily acquire the materials for the six initial tiers. The seventh upgrade material is always a special one, and there are few of them in a single playthrough. Some of them can be obtained by quests; some of them can be farmed. All of them can be found as a treasure.

Here is a list of the five top tier upgrade materials and where to find them.

• A King’s Orders: The easiest one is found in The Still Palace, in a chest right above its entrance.
• Drowned Tome: It is obtainable by killing The Unspeakable Deep. The only other place you can get this is at The Blackest Vault, left from the collapsible wooden platform.
• Twisted Heart: The only place you can acquire this is at Cran’s Pass as a treasure.
• Frozen Tome: It is obtainable at the Salt Alkymancery, in a hidden chest in the hallway to the right of the boss area. There is a platform obscured by stalactites in the foreground midway through the hallway.
• Charred Tome: It is obtainable at the Pitchwoods, on top of the Keepers of Fire and Sky sanctuary.
