Make sure you set it to Bike Damage off and Rider health on, under settings and it's at full simulation to make it quicker.
Start any session and then repeatedly crash into walls. Eventually this achievement will pop.

In Simulation start a new career and in Race Options put on Rider Health. Skip to the race and drive as fast as you can into the barrier. Repeat this until you injure the driver, then the trophy will unlock.
