The easiest way to do this is to wait until you answer all the questions correctly in one puzzle block, then intentionally answer the next block's questions incorrectly. The Movie Clips only have 3 questions, so that is your best bet for the "all correct" part of the achievement.
I did this backwards, actually. I answered all 3 questions of the movie clip incorrectly on purpose and then got 100% correct on the following block. I mention that not to show off, but to say from experience it doesn't matter which one you do first.
You do not have to do this on consecutive puzzle blocks. It just has to be in the same short game. So be sure to be mindful of how many more puzzle blocks there are in the round.
I actually got this one on the same game I got "No Margin For Error". Maximized my game time that way.

Thanks for verifying they don't have to be in the order in the achievement description. I can also verify they don't have to be consecutive; you can get 100% right on one, do another one half-and-half, then get one 100% wrong, and the achievement will pop.
What doesn't seem to work is skipping questions; if you don't answer, it's not counted as "wrong" for this one.
