You'll start the game as Max by default, you will unlock all his brothers along the course of the game. Most likely you will unlock Kenpo first (the one in the hospital) after completing his mini-quest. After doing so just click on Max and select "Pick Avatar", choose whichever you like and achievement unlocked.
